This form is designed to help me get to know you, your hair, and your goals so I can customize your look and set you up for success. Please fill out this form as thoroughly as possible — the more details you provide, the better I can understand your hair, your goals, and how to help you achieve your dream look. This also helps me determine if we’ll be a good fit to work together, since not every stylist-client pairing is the perfect match — and that’s okay!

Please note that at this time, I am not accepting new clients for all-over color root touch-ups, stand-alone toners or glosses, or stand-alone haircuts.

Due to the high volume of inquiries and incomplete forms I receive, I’m unfortunately unable to respond to submissions that don’t include enough information for me to assess your hair and goals. The more detail you share, the better I can help determine if we’re a good fit and provide an accurate estimate.

Once I review your form, I’ll reply and include a rough pricing estimate based on the information provided. Please note that final pricing can vary and is best determined after seeing your hair in person. In the meantime, you can visit my online booking page to view base pricing for most services.

Which best describes the length of your hair? Required

Short - Hair that typically falls above the chin or slightly below. Includes pixies, bobs, and cropped styles with minimal length and easy manageability.

Medium & Shoulder Length - Hair that falls around the shoulders or just below, offering versatility for styling while still being relatively low-maintenance.

Long - Hair that reaches from the collarbone to mid-back, providing plenty of styling options and volume for looks with movement and flow.

Extra Long - Hair that extends past the mid-back, often reaching the waist or beyond. It creates a dramatic, statement look but typically requires more care and maintenance.

Which best describes your hair thickness and density? Required

Very Fine - Hair strands are delicate, lightweight, and often silky, with minimal natural volume. Scalp may be easily visible when hair is parted.

Fine - Stands are thinner than average but have a bit more body than very fine hair. Volume is moderate but still tends to lay flat without styling.

Medium - Hair has an average stand thickness and density, offering a balance of volume and manageability. It generally holds styles well and looks full without feeling overly heavy.

Thick - Strands are noticeably more substantial, with a dense overall feel. Hair has natural volume and fullness but can require extra product or time to manage and style.

Extremely Thick - Hair is very dense and full, with coarse or heavy strands that create maximum volume. It can feel weighty and may require specialized techniques and products for control and styling.

Which best describes your hair texture? Required

Straight - Naturally smooth and sleek from root to tip with minimal natural wave or curl. It tends to reflect light well, often appearing shiny and polished.

Wavy - Hair with a gentle “S” pattern that falls between straight and curly. It has soft movement and can range from loose, beachy waves to more defined bends throughout the strands.

Curly - Defined curls that form spirals or ringlets, adding natural body and volume. Curl patterns can range from loose, bouncy loops to tighter, springier curls, each with its own unique shape and texture.

Coily - Tight, small curls or zig-zag patterns with lots of natural volume and texture. Coily hair tends to be dense and springy, with beautifully defined shapes that can vary from soft to structured.

Very Tight Coily - Very tight, compact coils or zig-zag patterns with a soft, cotton-like texture. This hair type is versatile and full of natural volume, with a range of coil sizes and densities that make every head of hair beautifully unique.

Which best describes the current health of your hair? Required

Very Damaged - Dry, brittle, and fragile with a dull, lifeless appearance. Feels rough or straw-like to the touch, breaks easily, and often has split ends and heavy frizz.

Moderately Damaged - Lacks shine and smoothness with noticeable frizz and dryness, especially on the ends. Feels coarse or rough in spots and tangles more easily, with color that may look uneven or faded.

Slighty Damaged - Mostly healthy overall but showing early signs of dryness or minor frizz from heat, color, or environmental stress. Feels fairly smooth with a few weaker, rougher areas, especially toward the ends.

Healthy - Shiny, soft, and smooth with minimal frizz and natural movement. Feels hydrated, flexible, and strong while holding styles well without excessive effort.

Very Healthy - Glossy, vibrant, and full of life with an effortlessly polished look. Feels silky, strong, and luxurious to the touch, with minimal tangling and maximum manageability.
